- Official high school transcript or copy of GED.
- High School Counselor Evaluation Form. If within three years of graduation, please ask the counselor to send the completed form directly to the University of Evansville Office of Admission.
- Dean of Students Recommendation Form. This form should be completed by the dean of students at the college or university you most recently attended, regardless of whether or not college credit was earned. Please ask the dean to send the completed form directly to the University of Evansville Office of Admission.
- Letter of Recommendation.
- Personal Statement.
