Transcript Request

Students may obtain a certified statement of their academic record from the Office of the Registrar upon written request with signature. We will accept transcript requests in person, by mail or by fax. If you plan to request for a transcript in person, please bring a picture ID for us to verify your identity. We will not release your transcript to anyone else unless they have a signed letter from you authorizing the release of your transcript.

If you are planning to request for a transcript through the mail or by fax, please print a copy of the transcript request form, complete it and submit it to:

There is no charge for transcript requests. Every effort is made to issue transcripts the day requests are received. At the end of the semester, however, approximately five days are required to post grades and issue transcripts for students who have just completed courses. Transcripts will not be released if the student has an unpaid financial obligation to the University or if there is an unresolved disciplinary action against the student.
