- Official transcript(s) from every post secondary school you have attended
- SAT or ACT score (these scores are most often located on the high school transcript)
- Recommendation form from the Office of the Dean of Students where you have taken the majority of your classes or from the university that you have attended most recently (some students may need to submit two recommendation forms)
- Optional personal statement
What if I am applying to the Doctor of Physical Therapy Program?
Applications to this professional program are accepted beginning in August of the academic year prior to the anticipated date of entry. Interviews for qualified candidates will begin in October. Entry into the professional program is competitive and a special application and interview are required. Applicants are admitted on a rolling basis. Once 40 seats are filled, further qualified applicants will be placed on an alternate list.
