In order to evaluate your experience, the following forms must be completed and turned in to the Center for Career Development. Additionally, all experiential education documentation is managed in Handshake for both students and employers.

Co-op/Internship Semester Work Report
This report is to be turned in at the end of your co-op or internship experience. In the report you will describe the objectives set for your internship or co-op experience and your accomplishments.
