We are here to train the next generation of mental healthcare professionals while providing exceptional care to our community.

If you would like to request our services, getting started is a simple process:

  1. Download our New Client Application
    Please print the PDF and fill it out completely.

  2. Submit your form.
    Because we require physical copies, please return your completed form to us via Fax, Mail, or In-Person Drop-off.

  3. What Happens Next?
    Once we receive your form, your name will be placed on our active waitlist. Wait times vary depending on current demand. As soon as one of our clinicians has an opening that matches your care needs, they will pull your application from the waitlist and you will receive a call to schedule your intake appointment.

Insurance and Payment Information

Pricing and insurance coverage vary depending on the provider and service you receive. Please contact the clinic directly regarding estimates of our fees.

  • Medication Management: All medication management services can be billed to insurance, provided our clinic is in-network with your insurance provider.
  • Psychological Assessments: Please note that we do not bill insurance for psychological assessments. These are offered on a self-pay basis; contact us for current rates.
  • Sliding Scale Options: We offer a sliding fee scale based on your income and household size. You can request this financial assistance at the time of your visit.
  • Medicaid Users: We currently accept Indiana Medicaid only. We are unable to accept out-of-state Medicaid plans at this time.
