Immunization Exemptions

The University of Evansville will consider a request for exemption under the following circumstances. Please note, if an exemption is filed, a verified status in Med+Proctor is still required.

Requesting an Exemption

It is the student's responsibility to complete the required exemption forms and return it to the Health Center. Students who file for an exemption, must still complete the remaining requirements and complete your Med+Proctor profile. The university will consider each exemption on a case-by-case basis. Students who are granted exemption must execute all necessary documentation required. Students who are denied exemption will need to complete all the required vaccinations in order to remain at University of Evansville. Exemption forms are due before the start of classes. Contact the Health Center should you require Medical or Religious exemption forms.

Medical Necessity/Contraindication

A Medical Exemption Form must be completed in addition to a written statement by a health care provider indicating the nature and probable duration of the medical condition or circumstances that contraindicates such immunization(s), identifying the specific vaccine(s), which could be detrimental to the student's health.

Religious Objection

A Religious Exemption Form must be completed and signed by the student. The university will consider granting an exemption based on a written statement by the student stating the specific religious beliefs on which the opposition to the required vaccination(s) is based and the theological basis for such belief. In the state of Indiana, “philosophical objection” is not a valid reason for exemption.

Quarantine Policy

In the event of an outbreak of a vaccine-preventable communicable disease on or near the campus, students with exemptions on file will be required to leave the campus until it is deemed safe for them to return by the county Health Department and Health Center.
