This checklist is provided to help aid you in completion of your health requirements. Before submitting your forms, please review the items below for completion to ensure you have met all requirements to attend the University of Evansville.

• Register your new account with Med+Proctor.
• Download the following forms and follow the directions provided. Select the forms specific to you:
  
  • Forms for Undergraduate General Population Students
  • Forms for Master of Physician Assistant Program Students
  • Forms for Doctor of Physical Therapy Students
  • Forms for Doctor of Nurse Anesthesia Practice Students
• Upload the certified Immunization certificate - Other forms of documentation showing proof of your immunizations and TB testing will be accepted as long as your name and date of birth are located on the document.
  
• Immunizations are up to date and compliant according to Indiana State and University of Evansville guidelines.
  • MMR – two doses required
  • Tdap - within 10 year
  • Meningococcal - 21 years of age or younger, 1 dose on or after 16th birthday
  • TB Test - within 6 months of start of semester (TB skin test or blood test)
  • Graduate programs have additional requirements, see forms
• Verified Status in Med+Proctor
